If you are submitting it as an entry and it's generating as an exit, then chances are you have the incorrect template configured.
If you can navigate to Settings > PIM Admin > Inspectors and Templates and then tap on your name and then tap Condition Report Template and from the list tap your state formatted template from one in this list below.

and then tap Done.
If your company has customised a template with requested changes you will see the state-based ones and then your company name after the word Entry. ie WA-PCR-Entry-PIM.docx. If you see one relating to your company name tap on that one, and then tap Done.
Try submitting it again, and if it still does it, just refresh the app by swiping upwards, and then swipe again to close the PIM App. Go back into PIM from your Home screen and submit the report again.
